Choking Hazards in Toys: What the Small Parts Test Actually Checks

The federal small parts cylinder is real, and it still missed most of the choking deaths in a 2017 recall review. What it tests, and how to check a toy at home.

Every toy sold for a child under three has already passed a physical test: a piece is dropped into a cylinder, and if it fits all the way inside, the toy fails and cannot be sold in that form. The rule is federal, it is specific, and it is the reason a rattle has a fat handle instead of a slim one. It is also, on its own evidence, not enough.

Here is what the small parts test actually measures, where the standard behind it comes from, and what a 2017 review of recalled products found when it checked the test against real incidents.

The test itself

The small parts cylinder is defined in 16 CFR Part 1501, the federal regulation the Consumer Product Safety Commission uses to identify toys that present a choking, aspiration or ingestion hazard for a child under three. The cylinder measures 2.25 inches deep by 1.25 inches across, built to approximate the fully expanded throat of a young child. Any piece of a toy, including a piece that can be broken or pulled off with reasonable force, that fits entirely inside that cylinder makes the toy a small-parts hazard for that age group.

A toy that fails is not necessarily banned outright. It can still be sold, but it must carry a choking-hazard warning label naming the minimum safe age, and it is barred from being marketed toward children under three regardless of the label. That labeling requirement is what puts the small yellow warning triangle on the box, and it is worth reading rather than skimming, because it is the one piece of text on packaging that reflects an actual physical test rather than a marketing department's judgment.

The regulation sits underneath ASTM F963, the broader consumer safety specification for toys that 16 CFR Part 1250 makes mandatory for anything sold in the United States as a plaything for a child under 14. ASTM F963 covers far more than choking: sharp points and edges, lead and phthalate limits, magnet strength, battery compartment security, sound pressure levels, and the age grading printed on the box. The small parts cylinder is the piece of that standard specifically aimed at what a baby's mouth can do to a toy, and what a toy's smallest piece can do to a baby's airway.

What the test does not check

The cylinder tests a piece in isolation, at rest, in the shape it was manufactured. It does not test what a determined toddler's teeth, drop height, or repeated bending can do to a toy over months of use. A toy that passes on day one because no single piece fits the cylinder can still fail structurally later, shedding a smaller piece the original test never saw.

A 2017 study published in the International Journal of Pediatrics, by researchers including Nancy Cowles of Kids In Danger, reviewed 303 recalled children's products issued between 2003 and 2014 for choking, aspiration or ingestion hazards. The recalls involved 211 documented incidents. Choking accounted for close to half of those incidents, and toys were the largest single product category, ahead of clothing and nursery items.

The finding that matters most for a parent checking a toy at home: 17.1 percent of the recalled products involved a part larger than the small parts cylinder, meaning the piece would have passed the federal test and still choked or nearly choked a child. Three of the four choking deaths documented in the review involved objects that exceeded the cylinder's dimensions, including a toy nail from a play tool bench.

The authors' argument was not that the cylinder is measured wrong today. It is that the cylinder's dimensions trace back to research on children's airways from the 1980s, and that a test built around a single fixed size cannot account for how a piece actually behaves once it separates, gets coated in saliva, or lodges past the point where a rigid measurement predicted it would stop.

None of this makes the standard useless. It removes a large share of hazards before a toy reaches a shelf, and the labeling requirement gives a parent real information the small print did not have to include. It means the cylinder is a floor, not a guarantee, and a toy that clears it by a wide margin is a different proposition from one that clears it narrowly.

How to check something already in the house

An empty toilet-paper tube is a close, cheap stand-in for the official cylinder, close enough to be genuinely useful for a quick check, and it is why the trick shows up in pediatric offices as often as it does. If a piece fits all the way through, treat it as a hazard for a child under three regardless of what the box says, and regardless of whether the toy is intended for an older sibling.

Check under tension, not just at rest. Pull on eyes, wheels, buttons and any glued-on decoration with real force, the kind a determined one-year-old applies without thinking about it. A piece that will not come off with a firm tug is a different risk than one that separates the first time it is chewed.

Check after damage, not just when new. A cracked rattle, a chewed board book corner, or a stuffed animal with a seam coming apart has left the condition it was tested in. The federal test evaluated the toy as manufactured; it did not evaluate the toy six months and four teeth later.

Watch for items that were never toys at all. The review's own worst cases were largely non-toy small objects, including button batteries, coins, and food items, that a small-parts warning label would never have appeared on because the object was never marketed to a child in the first place. A toy budget conversation is not the same conversation as a household hazard sweep, and a house with an older sibling needs both.

The age grading, and what it is actually telling you

A toy marked "3+" on otherwise simple, non-technical packaging is almost never saying the toy is too advanced for a two-year-old. It is a choking-hazard classification, and reading it as a difficulty rating is one of the more common and avoidable mistakes in a first-year toy budget. Follow the label exactly rather than judging by hand whether a piece looks big enough, because a piece that looks safe and a piece that fits the cylinder are not always the same piece.

The age grading requirement comes from the same ASTM F963 standard, and it interacts directly with the small parts rule: a toy intended for a wider age range, say 18 months to 5 years, has to be safe for the youngest child in that range, which is one reason multi-age toy sets sometimes have surprisingly large minimum pieces.

What follows for buying

A wide margin beats a narrow pass. A rattle handle that is visibly, generously too fat for the cylinder is a better bet than one that is a millimeter over. Manufacturers test to the letter of the standard, and you are free to prefer more room than the letter requires.

Fabric and stuffed pieces need the same scrutiny as hard plastic. Bean-filled weights, plastic safety eyes, and small appliques on soft toys are common recall categories, and a soft toy is not automatically a gentle one.

Check the CPSC recall database by brand and product name before a purchase, not just after an incident. A toy currently for sale is not automatically a toy that has never been recalled in an earlier version.

Treat older siblings' toys as a distinct hazard category, since a toy correctly rated for a 4-year-old is frequently within reach of a crawling sibling, and the rating on the box was never evaluated against that household.
